Reflectance confocal microscopy and optical coherence tomography for the diagnosis of bullous pemphigoid and pemphigus and surrounding sub-clinical lesions

Mandel VD, et al. - During this research, the characteristics of bullous pemphigoid (BP) and pemphigus as revealed via reflectance confocal microscopy (RCM) and optical coherence tomography (OCT) were comprehensively assessed to gain a rapid non-invasive bed-side diagnosis. Researchers also intended to gauge the detectability of clinically non-visible lesions. It was reported that RCM and/or OCT imparted beneficial information for a rapid diagnosis of BP and pemphigus and for the detection of the biopsy site. The use of RCM plus OCT combined the higher resolution of RCM with the greater penetration depth of OCT. Hence, this combination was found to be optimal. Particularly in cases of sub-clinical lesions, OCT could serve as an optimal tool for treatment monitoring. Nonetheless, histopathologic and immunologic examinations were the established gold standard for arriving at the final diagnosis.
