Intraoperative iatrogenic retinal breaks in 23-gauge vitrectomy for stage 3 and stage 4 idiopathic macular holes

Yu Y, et al. - In this retrospective study, researchers examined features of intraoperative iatrogenic retinal breaks in 23-gauge vitrectomy for idiopathic macular hole and classified the breaks based on their causes to analyse the risk factors. Participants in the study were patients with stage 3 or 4 idiopathic macular hole who had 23-gauge vitrectomy in Beijing Tongren Hospital from July 2015 to August 2018. In total, 341 patients (364 eyes) were involved. The distribution of iatrogenic retinal breaks intraoperatively shows no preference for the superior or inferior region. Posterior vitreous detachment induction and peripheral vitreous cutting traction are major causes of the breaks, which classify them into two main types. When treated correctly, the presence of a lattice may not be one of the risk factors.
