KIMS performs 16-hr rare hybrid aortic aneurysm surgery on a farmer

Krishna Institute of Medical Sciences (KIMS), Hyderabad, on July 15 said that its vascular surgeons had performed a 16-hour rare hybrid aortic aneurysm surgery on a farmer.
 


A 50-year-old farmer’s life-threatening condition was discovered just in time when he came to KIMS from a village near Guntur with pain in the abdomen. Upon investigation, his scans showed that he had bulge of vessel measuring 9 centimeters from the chest to the abdominal aorta. A challenging 16-hour hybrid surgery was successfully performed by team of vascular surgeons led by Dr Narendranadh Meda and Dr Chandrasekhar in co-ordination with anesthesia team.

Speaking on the occasion, Dr Meda, Vascular and Endovascular surgeon, an expert in aortic-related diseases, said, “Patient was a known hypertensive, on investigation with CT, we found that he had a very large thoraco-abdominal aortic aneurysm (Main blood vessel carrying the blood from heart to the body) measuring nine centimeters in diameter was seen with impending rupture, involving the arteries supplying the bowel and kidneys.

Generally, Aneurysms greater than five centimeters are considered dangerous and need treatment.” His condition required an extensive surgery and we have opted for open De-branching (reconnecting all blood vessels) bypass surgery to replace portions for two visceral vessels which supply liver, bowel and two renal vessels and supply kidneys with a Dacron woven graft with reversing supply from normal infrarenal aorta, he said.

Once the blood supply to the visceral and renal arteries was secured a stent was placed in the same setting to exclude the aneurysmal segment,” he added. It took 16-hours for us to complete the surgery, Dr Meda said. People suffering from such advanced Aortic Aneurysm should have an early diagnosis made, he cautioned. In most cases, abdominal aortic aneurysms cause no symptoms and are found when being evaluated for another medical condition. If there is a family history of aneurysm and feel sudden severe pain in abdomen or back, one should seek immediate care.

Although the exact cause of abdominal aortic aneurysms is unknown, they can be hereditary. Factors Like age, hypertension, smoking, infection, trauma and family history may play a role,” Dr Meda said. The doctor suggested a routine screening ultrasound of abdomen which can pick up aortic aneurysm in a healthy asymptomatic patient.
