Usefulness of right ventricular volumetric and afterload indices for risk stratification in patients with tetralogy of Fallot

Egbe AC, Najam M, Banala K, et al. - In this cross-sectional analysis, researchers examined adults with prior tetralogy of Fallot repair in order to compare the correlation of afterload-adjusted right ventricular (RV) volumetric indices, vs RV volumetric indices alone, with disease severity in these patients who received care at Mayo Clinic, 2002-2015. Atrial arrhythmia and/or impaired exercise capacity defined disease severity. Afterload-adjusted RV volumetric indices were constructed by indexing these indices to RV systolic pressure (RVSP) as follows: RV end-diastolic volume/RVSP, RV end-systolic volume (RVESVi)/RVSP, and RV ejection fraction/RVSP. The only RV volumetric parameter that was found to have a relation with disease severity was RVESVi, with area under the curve 0.612. Findings revealed a better correlation of afterload-adjusted RV volumetric indices with disease severity. Improved risk stratification in this population may potentially be done with the help of afterload-adjusted RV volumetric indices.
