Usefulness of dynamic contrast-enhanced magnetic resonance images for distinguishing between pyogenic spondylitis and tuberculous spondylitis

Miyamoto H, et al. - Forty-five consecutive patients diagnosed with pyogenic spondylitis (PS) and 14 with tuberculous spondylitis (TB) were recruited in this study by the researchers in order to explain the usefulness of dynamic contrast-enhanced (DCE)-magnetic resonance image (MRI) for differentiating PS and TB. DCE-MRI included serial six sagittal images that were imaged every 20s following intravenous gadolinium administration. Among PS and TB, degree of enhancement, presence of an epidural abscess, presence of necrosis in the vertebra, presence of enhancement in disc lesion, the pattern of diffusion, and maximum contrast index were analyzed and contrasted. The degree of enhancement, percentage of epidural abscess, and percentage of necrosis in the vertebra with no statistical variation were observed. For PS and TB, the highest contrast index, percentage of the diffusion pattern from the disc, and percentage of the enhanced disc were 108.1 ± 22.3 and 78.2 ± 35.6 s, 89.3% and 0%, and 53.6% and 0%, respectively, with statistical importance. Hence, a more sustained maximum contrast index, greater likelihood of diffusion pattern from the disc, and a higher likelihood of enhanced disc were concluded as more specific to PS than TB. Moreover, for a more precise diagnosis of PS and TB, this less invasive imaging technique was observed as beneficial.
