Unrecognised myocardial infarction and its relationship to outcome in critically ill patients with cardiovascular disease

Docherty AB, et al. - Researchers conducted this multicenter prospective cohort study in 11 intensive care units (ICUs) in the United Kingdom to determine the incidence of myocardial infarction (MI) and its association with long-term survival. Participants included 273 patients in the ICU with coexisting cardiovascular disease. For 10 days, troponin I was measured with a high sensitivity assay. For 5 days, ECGs were carried out daily and were analyzed by blinded cardiologists for dynamic changes. The diagnosis of myocardial “infarction,” “injury,” or “no injury” was done using the third universal definition of MI and by combining the data. In this study population, ~25% of patients were found to have undiagnosed MI, which was associated with lower long-term survival.
