Ulcer bleeding after band ligation of esophageal varices: Risk factors and prognosis

Dueñas E, Cachero A, Amador A, et al. - In this investigation, researchers determined the incidence, outcome and risk factors of post-banding ulcer bleeding, a rare complication of endoscopic band ligation of esophageal varices with high morbidity and mortality. Data were prospectively gathered for cirrhotic patients with acute variceal bleeding over a 6-year period, and all band ligation sessions conducted were analyzed retrospectively. The research involves 521 band ligation sessions performed on 175 patients. Data reported that mortality during the bleeding episode was 23.8%. According to findings, post-banding ulcer bleeding is a rare but serious complication of esophageal banding. The risk of post-banding ulcer bleeding is greater for patients with hepatocellular carcinoma, poor liver function and low beta-blocker dose. In case of active bleeding at endoscopy, aggressive treatment should be considered. Treatment failure or death was correlated with active bleeding or adherent clots at the time of endoscopy.
