The outcomes of pulmonary hypertension patients with severe aortic stenosis who underwent surgical aortic valve replacement or transcatheter aortic valve implantation

Al-khadra Y, Darmoch F, Pacha HM, et al. - Using the National Inpatient Sample database, researchers examined patients who were diagnosed with pulmonary hypertension (PH) and underwent transcatheter aortic valve implantation (TAVI) surgical aortic valve replacement (SAVR) for aortic valve stenosis from 2011 to 2014, to compare the outcomes between TAVI and SAVR. They identified 36,786 patients who received a diagnosis of PH and had undergone an intervention for aortic valve stenosis; TAVI (n = 9,560, 26%) or SAVR (n = 27,225, 74%). The TAVI group comprised older patients, more women, and less African-American patients. Although co-morbidities were comparable between both groups, a higher prevalence of congestive heart failure, chronic pulmonary disease, renal failure, peripheral vascular disease, coronary artery disease, and previous stroke was observed in the TAVI group vs the SAVR group. Both groups had similar in-hospital mortality but lower cardiac, respiratory and bleeding complications were reported in relation to TAVI vs SAVR.
