The independent effect of various cross-linking treatment modalities on treatment effectiveness in keratoconus

Godefrooij DA, et al. - Researchers performed this prospective longitudinal cohort study to determine 1-year results of employing various corneal cross-linking (CXL) techniques for treating keratoconus. This investigation was conducted at a tertiary academic medical center. They observed for 1 year 670 eyes of 461 patients having progressive keratoconus who underwent CXL. They evaluated eight combinations of CXL modalities, comprising 2 different CXL techniques (transepithelial or epithelium-off), 7 riboflavin formulations, and 2 ultraviolet-A protocols (conventional 3 mW/cm2 or accelerated 9 mW/cm2). The reference group comprised of patients treated using the Dresden protocol. Concerning keratoconus progression control, the observed reduced efficacy seemed to be related to transepithelial CXL (TE-CXL), the use of Meran riboflavin, and applying the accelerated irradiation protocol. Re-treatment was needed in one-third of cases managed using TE-CXL.
