The effect of baricity on maternal cardiac output of intrathecal bupivacaine for elective cesarean section. A randomized study

Limratana P, Kiatchai T, Somnuke P, et al. - Given the association of hemodynamic instability induced by intrathecal administration of local anesthetics for cesarean delivery with adverse maternal and fetal outcomes, researchers here compared the hemodynamic profiles of healthy term parturients randomized to receive intrathecal plain or hyperbaric bupivacaine for cesarean delivery anesthesia using a noninvasive suprasternal Doppler cardiac output monitor. The randomized healthy term parturient patients were assessed for following hemodynamic parameters: blood pressure, heart rate, cardiac output (CO), and systemic vascular resistance. In addition, they were assessed for the incidence of hypotension, vasopressor use, and fetal outcomes. Outcomes revealed no significant differences in hemodynamic profiles after intrathecal plain or hyperbaric bupivacaine administration in these parturients undergoing spinal anesthesia for cesarean delivery. They suggest no influence of bupivacaine baricity on maternal cardiac output and indicate a possible association of hyperbaric bupivacaine with more reliable surgical anesthesia.
