Ten-year follow-up of stemmed hemiarthroplasty for acute proximal humeral fractures

Amundsen A, Brorson S, Olsen BS, et al. - Ten-year patient-reported outcomes after hemiarthroplasty for acute proximal humeral fractures, as well as the cumulative revision rate and risk factors for an inferior patient-reported outcome, are reported in this study. From the Danish Shoulder Arthroplasty Registry, data on 1,371 hemiarthroplasties for acute proximal humeral fractures between 2006 and 2010 were obtained. Of these patients, 549 (40%) were alive and were sent the Western Ontario Osteoarthritis of the Shoulder (WOOS) questionnaire at nine to 14 years after primary surgery; mean age at surgery: 67 years (24 to 90); 445 (81%) patients female. A total of 364 (66%) patients returned a complete questionnaire at a mean follow-up of 10.6 years (8.8 to 13.8). In this largest long-term follow-up study of acute proximal humeral fractures treated with hemiarthroplasty, a low revision rate and an acceptable ten-year patient-reported outcome were reported. As no information was available regarding the patients who died or did not return a complete WOOS score, interpretation of the patient-reported outcome should be done with caution. The long-term outcome and revision rate suggest that when non-surgical treatment is not possible, hemiarthroplasty represents a valid alternative.
