Systematic quantification of histologic ventricular fibrosis in isolated mitral valve prolapse and sudden cardiac death

Han HC, Parsons SA, Curl CL, et al. - Researchers systematically quantified left and right ventricular fibrosis in patients with isolated mitral valve prolapse (MVP) and sudden cardiac death (SCD) (iMVP-SCD), whereby other potential reasons of death were eliminated, vs a control cohort. From the Victorian Institute of Forensic Medicine, Australia, people with iMVP-SCD were selected and were matched for age, gender, and BMI to control cases with noncardiac death. Increased left ventricular (anterior, lateral, and posterior) and interventricular septum fibrosis was detected in the iMVP-SCD group but similar amounts of right ventricular fibrosis was present vs controls. In iMVP-SCD, significantly higher left ventricular fibrosis was detected in the lateral and posterior walls vs the anterior wall and interventricular septum. Overall, the findings suggest the importance of nonuniform left ventricular remodeling, with both localized and generalized left ventricular fibrosis, in SCD pathogenesis in people having MVP.
