Surgical morbidity of total laparoscopic hysterectomy for benign disease: Predictors of major postoperative complications

Casarin J, Cromi A, Bogani G, et al. - Researchers aimed at determining the rate of perioperative complications, as well as investigating the risk factors of 30-day major (Clavien-Dindo ≥ 2) postoperative complications, of total laparoscopic hysterectomy (TLH) for benign disease. They performed an uncontrolled single-center single-arm retrospective study and included a total of 3,090 patients in the study. Outcomes revealed acceptable major postoperative complications rate in correlation with undergoing total hysterectomy performed via laparoscopy. Multivariable analysis revealed the following as independent risk factors for major postoperative complications: endometriosis, the need for conversion to open surgery, and the occurrence of any intraoperative complication. Based on outcomes, they recommend making a huge effort to reduce the occurrence of intraoperative complications and the necessity for conversion to open surgery. They emphasize counselling patients undergoing TLH for endometriosis regarding the raised risk of major postoperative events.
