Supervised injection facility use and all-cause mortality among people who inject drugs in Vancouver, Canada: A cohort study

Kennedy MC, et al. - Considering the previous studies demonstrating the role of supervised injection facilities (SIFs) in decreasing various harms associated with injection drug use, including accidental overdose death, researchers here examined how SIF use influence all-cause mortality among people who inject drugs (PWID) in Vancouver, Canada. In this study, a community-recruited cohort of 811 PWID in Vancouver, Canada, was prospectively followed for a median duration of 6 years. Findings revealed a high burden of premature mortality in this cohort of PWID. During follow-up, death of a total of 112 participants (13.8%) was reported, indicating a crude mortality rate of 22.7 (95% confidence interval 18.7–27.4) deaths per 1,000 person-years and a median of 34 years of potential life lost (interquartile range 27–42) per death. Further, they observed a reduced risk of dying among individuals who reported using supervised injection facilities on an at least weekly basis vs those who reported less than weekly or no use of this health service. This association remained significant following adjustment for potential confounders comprising age, gender, HIV seropositivity, unstable housing, at least daily cocaine injection, public injection, incarceration, enrolment in addiction treatment, and calendar year of the interview.
