Selective vs stepwise removal of deep carious lesions in permanent teeth: A randomized controlled trial from Egypt—an interim analysis

Labib ME, et al. - Through a randomized controlled, unicentre, clustered two-arm superiority trial done at the dental clinic complex of Misr International University, Cairo, Egypt with 115 individuals with 132 teeth having occlusal or occlusal-proximal deep carious lesions and sensible pulps, without permanent or severe pain, researchers contrasted success, survival and initial and longer-term costs of selective excavation (SE) and stepwise excavation technique (SW) in permanent teeth with deeply carious permanent teeth. It was hypothesized that SE would exhibit significantly greater success than SW. It was discovered that SW was more expensive initially, essentially as the second SW step needed more treatment time though also as additional materials (eg, GI, anesthesia) were required. Nevertheless, long-term and analyzing that SE was performed in two steps, no meaningful variations were detected. If eliminating the supplementary traveling and indirect/opportunity costs of the second SE step, SW was also more expensive long-term. In conclusion, following 1 year of this randomized trial, success and survival did not vary significantly between SE and SW. When SE was performed in two visits, the total prices of SE were comparable to those of SW. Dentists may decide either SE or SW for treating deep carious lesions, while on the basis of this study, there is no robust support to prefer SW over SE for lesions extending > 2/3 dentine.
