Risk factors associated with positive resection margins in patients with adrenocortical carcinoma

Skertich NJ, Tierney JF, Chivukula SV, et al. - Given a link between positive resection margins and worse survival after surgical management of adrenocortical carcinoma (ACC), researchers investigated the risk factors for positive margins post-resection. Using the NCDB, they selected 1,973 patients with ACC, 217 (11.0%) with positive margins. They used Chi-square tests to compare patients with positive vs negative resection margins after surgery. The factors that conferred risk for positive margins were: extra-adrenal extension, lymph node metastases, and distant metastases, as revealed in the multivariable analysis. Patients who underwent an open vs minimally invasive procedure exhibited no significant difference in margin status. Overall, no modifiable risk factors were detected, but the longest survival was reported in patients with positive margins receiving adjuvant radiation or chemo-radiation.
