Recent metformin adherence and the risk of hypoglycaemia in the year following intensification with a sulfonylurea

Min JY, et al. – Researchers conducted this study to determine if recent low adherence to metformin monotherapy is associated with hypoglycemia after addition of a sulfonylurea. Participants included veterans who filled a new prescription for metformin from 2001-2011 and intensified treatment with a sulfonylurea after ≥ 1 year of metformin use. From pharmacy data using the proportion of days covered in the 180-day period before intensification, metformin adherence was calculated. Researchers reported that the median rate of treatment adherence was 87 %, and 43% had adherence < 80%. When adherence was modelled as a continuous variable, the risk of hypoglycemia was comparable across all levels of adherence. Past low adherence to metformin monotherapy was not identified to be associated with hypoglycemia after intensification with a sulfonylurea.
