Predictors of adverse pathologic features after radical prostatectomy in low-risk prostate cancer

Park JW, et al. - In this retrospective review, researchers analyzed predictors of adverse pathologic features (APFs) after radical prostatectomy (RP) in low-risk prostate cancer (PC) using classification under the current criteria. PC with clinical T1–T2a, Gleason score ≤ 6, and prostate-specific antigen (PSA) levels < 10 ng/mL defined low-risk PC. Univariable and multivariable logistic regression analyses were used, as well as receiver operator characteristics to predict APFs. A significant correlation of positive surgical margins (PSM) with preoperative high PSA levels and number of positive cores obtained was shown. A significant correlation of extracapsular extension (ECE)/seminal vesicle invasion (SVI) with PSA levels and number of positive cores was also observed. Overall, PSA > 4.5 ng/mL and number of positive cores > 2 in low-risk PC were found to be related to the presence of APFs. Careful consideration of patients with such records was recommended to provide active surveillance.
