Predicting survival after liver transplantation in patients with hepatocellular carcinoma using the LiTES-HCC score

Goldberg D, Mantero A, Newcomb C, et al. - In the US and Europe, liver transplant priority is based on binary tumor criteria (eg, Milan) for patients with hepatocellular carcinoma (HCC) to expedite transplant for patients with "acceptable" post-transplant outcomes. To overcome limitations of these binary criteria, newer risk scores (eg, Metroticket, HALT-HCC)were created, but these focus only on HCC-related pretransplant variables, making them insufficient to be used for waitlist priority. In this study, a risk score to predict posttransplant survival for HCC patients has been developed using HCC- and non-HCC related variables. Researchers conducted a retrospective cohort study of adult deceased-donor liver transplant (DDLT) recipients with HCC using national registry data. Eleven variables were selected in the final model from among 6,502 adult HCC LT recipients. Using the LiTES-HCC score, patients could be identified on the basis of post-transplant survival among those meeting Milan and UCSF. Inclusion of HCC- and non-HCC related variables (eg, age, chronic kidney disease) in this score may make it possible for transplant professionals to prioritize patients with HCC in terms of predicted survival.
