Post-hospitalization dialysis facility processes of care and hospital readmissions among hemodialysis patients: A retrospective cohort study

Plantinga LC, et al. - Researchers used a retrospective cohort of electronic medical record (EMR) data connected with national registry data to assess the relationship of post-hospitalization processes of care at hemodialysis facilities with pulmonary edema-related and other readmissions. Factors related to a increased risk of pulmonary edema-related vs no readmission included documentation of the index admission, congestive heart failure, and home medications being stopped or changed in the EMR post-hospitalization, whereas, a 40% lower risk was reported in relation to lower post-dialysis weight (by ≥0.5 kg) after vs before hospitalization. These findings suggest a possible association of some interventions performed at the dialysis facility in the post-hospitalization period with reduced readmission risk, while other interventions may detect patients at higher risk for readmissions who could benefit from said interventions.
