Planned early delivery or expectant management for late preterm preeclampsia (PHOENIX): A randomized controlled trial
The Lancet Sep 05, 2019
Chappell LC, Brocklehurst P, Green ME, et al. - Via a parallel-group, non-masked, multicentre, randomized controlled trial conducted in 46 maternity units across England and Wales, researchers ascertained whether planned earlier initiation of delivery decreased maternal adverse outcomes without substantial worsening of neonatal or infant outcomes, in comparison with expectant management (usual care) in women (n = 901) with late preterm preeclampsia. There was robust evidence to recommend that planned delivery decreased maternal morbidity and severe hypertension in contrast with expectant management, with more neonatal unit admissions associated with prematurity, however, there was no indicators of higher neonatal morbidity. This trade-off should be addressed to women with late preterm preeclampsia to permit shared decision making on the timing of delivery.
