Peri-intubation cardiac arrest in the emergency department: A National Emergency Airway Registry (NEAR) study

April MD, Arana A, Reynolds JC, et al. - Researchers sought to report on the incidence of peri-intubation cardiac arrest via analyzing a multi-center emergency department (ED) airway registry, as well as described linked clinical characteristics. Prospectively obtained data (National Emergency Airway Registry) comprising ED endotracheal intubations (ETIs) of individuals > 14 years old from 2016 to 2018 were used in this secondary analysis. Selection criteria were met by 15,776 individuals, 157 of whom (1.0%, 95% CI 0.9-1.2%) experienced peri-intubation cardiac arrest. This suggests a rare occurrence of peri-intubation cardiac arrest among patients undergoing ETI in the ED. Patients with pre-intubation shock or hypoxemia were noted to be at greater risk of developing arrest. They suggest a necessity for prospective trials to ascertain if a protocol to optimize pre-intubation haemodynamics and oxygenation reduces the risk of peri-intubation cardiac arrest.
