Patiromer for treatment of hyperkalemia in the emergency department: A pilot study

Rafique Z, et al. - Researchers examined the potential efficacy and safety of oral patiromer in the treatment of acute hyperkalemia in the emergency department (ED). In this single-center, randomized, open-label convenience sample pilot study, they randomized adult patients with end-stage renal disease and a serum potassium level of ≥ 6.0 mEq/L to the standard of care (SOC) or one dose of 25.2 g oral patiromer plus SOC. The patients provided blood samples and electrocardiograms at enrollment and at 1, 2, 4, and 6 hours thereafter. The final analysis comprised 30 patients, 15 in each group. The analysis revealed a reduction in serum potassium within 2 hours following receipt of a single dose of 25.2 g of oral patiromer but at 6 hours, no difference was observed. This study for the first time indicated a possible role of patiromer in the acute management of hyperkalemia; however, more rigorous studies are required.
