Palatine tonsil SUVmax on FDG PET-CT as a discriminator between benign and malignant tonsils in patients with and without head and neck squamous cell carcinoma of unknown primary

Pencharz D, et al. - In order to determine useful diagnostic thresholds, researchers conducted a study to analyze the maximum standardised uptake value (SUVmax) ratio between tonsils in patients with and without tonsillar carcinoma. Positron emission tomography (PET)/computed tomography (CT) examinations of suspected head and neck squamous cell carcinoma (SCC) patients and controls were reviewed retrospectively from April 2013 to September 2016. For the presence of carcinoma, an SUVmax ratio between tonsils of greater than 1.6 had 100% positive predictive value. Some malignant tonsils have normal FDG uptake and therefore PET/CT should not be used to exclude tonsillar cancer. In non-malignant tonsils, minor asymmetrical uptake is often seen and does not necessarily require further investigation. Other centres might need to develop their own cut-offs due to the single centre nature of this study and the recognised variation in SUV measurements between PET scans.
