Outcomes of transcatheter aortic valve implantation in patients with chronic and end-stage kidney disease

The American Journal of Cardiology Dec 01, 2021

Hahn J, Ul Hassan Virk H, Greason K, et al. - Findings demonstrate independent associations of both atrial fibrillation (AF) and mitral stenosis (MS) with increased mortality in chronic kidney disease (CKD) patients undergoing transcatheter aortic valve implantation (TAVI).

  • A higher risk of aortic stenosis has been noted in patients with CKD and end-stage kidney disease (ESKD) and there exists limited data on TAVI in these patients.

  • From a large urban healthcare system, clinical data of patients with ESKD (n=84) and CKD (n=643) who had TAVI from 2008 to 2018 were analyzed.

  • The overall in-hospital mortality in CKD group was 5.1% and those with ESKD had an overall mortality rate of 11.9%.

  • Post-multivariable analysis, independent links of both MS (adjusted odds ratio (OR) 3.92) and AF (adjusted OR 2.42) with mortality were identified in CKD patients.

  • A link, albeit not statistically significant, was observed between MS and increased mortality in ESKD patients (adjusted OR 2.01).
