Outcomes of left main revascularization in patients with acute coronary syndromes and stable ischemic heart disease: Analysis from the EXCEL trial

Doucet S, et al. - In the randomized Evaluation of XIENCE versus Coronary Artery Bypass Surgery for Effectiveness of Left Main Revascularization (EXCEL) trial, researchers determined if the acuity of presentation distinguished patients who had a differential benefit from percutaneous coronary intervention (PCI) vs coronary artery bypass grafting (CABG). Using multivariable Cox models, they examined patients with low or intermediate SYNTAX scores to evaluate the interaction between the acuity of presentation, type of revascularization, and outcomes. They found that, in patients with left main coronary artery disease undergoing revascularization, outcomes were not forecast by the acuity of presentation. Also, patients who derive greater event-free survival from PCI vs CABG were not distinguished by the acuity of presentation.
