Outcomes of free vascularized fibular graft for post-traumatic osteonecrosis of the femoral head

Richard MJ, DiPrinzio EV, Lorenzana DL, et al. - This study was conducted to assess the longevity and outcomes of free vascularized fibular graft (FVFG) for traumatic hip injuries resulting in osteonecrosis of the femoral head (ONFH). Between 1980-2006, researchers conducted a retrospective review of the institutional database of patients who underwent FVFG for post-traumatic ONFH and had a minimum follow-up of 5 years. A total of 72 hips in 68 patients were included in the study. The findings demonstrate improvement in Harris Hip scores in surviving FVFG and an acceptable overall total hip arthroplasty conversion rate at mid to long -term follow-up in Urbaniak stage I through IV hips. The outcomes reveal that FVFG remains a viable option for treatment in younger patients with pre-and post-collapse (stage IV) ONFH lesions secondary to hip trauma.
