Operative vs. conservative treatment of small non-traumatic supraspinatus tears in patients over 55 years: over 5-year follow-up of a randomized controlled trial

Kukkonen J, Ryosa A, Joukainen A, et al. - This study was attempted to assess the difference in mid-term clinical and radiological outcomes in patients aged over 55 years. Researchers randomized a total of 180 shoulders with symptomatic, non-traumatic supraspinatus tears to one of the three cumulatively designed treatment groups: physiotherapy, acromioplasty and physiotherapy, and rotator cuff repair, acromioplasty, and physiotherapy. The results of this study exhibited that operative treatment is no better than conservative treatment with regard to small non-traumatic single tendon supraspinatus tears in patients over 55 years of age. It has been reported that operative treatment does not protect against degeneration of the glenohumeral joint or cuff tear arthropathy. For the primary initial treatment of this condition, conservative treatment was found to be a reasonable option.
