Oncologic and fertility impact of surgical approach for borderline ovarian tumors treated with fertility-sparing surgery

Marchette MD, et al. - A sum of 535 candidates were estimated to delineate the oncologic and fertility influence of surgical approach (open surgery vs laparoscopy) for borderline ovarian tumors (BOTs) managed with fertility-sparing surgery. Investigators observed 10-year recurrence rate of 23% for salpingo-oophorectomy (SO) and 31% for cystectomy (Cy) group in subjects with a unilateral tumor, whereas it was 62% and 72%, respectively, in cases with the bilateral tumor. No relationship between recurrence and surgical approach, type of surgery was noted. But the negative relationship with advanced stage and bilateral tumors were observed in multivariable analysis. They found a positive relation of fertility after surgery with previous pregnancy and negatively correlated with the number of surgical procedures. They suggested no impact of type of surgical procedures on recurrence rate or fertility. Although, a decline in fertility potential reported due to additional surgical procedures.
