Microwave ablation vs parathyroidectomy for secondary hyperparathyroidism in maintenance hemodialysis patients

Jiang B, et al. - Researchers assessed minimally invasive microwave ablation (MWA) vs total parathyroidectomy plus forearm autotransplantation for effectiveness and safety in secondary hyperparathyroidism (SHPT) patients who were undergoing maintenance hemodialysis (follow-up for 6 to 24 months after treatments). MWA group included 33 patients and 48 patients were included in parathyroidectomy group. These groups were compared by assessing serum intact parathyroid hormone (iPTH), calcium, and phosphorus levels as well as hoarseness, hypocalcaemia, and persistently low iPTH. At all follow-up times, significantly higher rates of achieving the recommended goal for iPTH were observed in the MWA group vs in the parathyroidectomy group. In univariate logistic regression analysis, the identified predictors were baseline levels of iPTH and patient age. Overall, MWA could be an alternate therapy to parathyroidectomy for SHPT.
