Lower on-treatment platelet reactivity during everolimus-eluting stent implantation contributes to the resolution of post-procedural intra-stent thrombus: Serial OCT observation in the PRASFIT-Elective study

Konishi A, et al. - Given the adverse coronary events observed in association with intra-stent thrombus (IS-Th) formed immediately after percutaneous coronary intervention (PCI), researchers assessed the impact of on-treatment platelet reactivity on IS-Th. They used participants in the PRASFIT-Elective, a multicenter study of PCI patients receiving prasugrel (20/3.75 mg, loading/maintenance dose) or clopidogrel (300/75 mg), with aspirin (100 mg), for this study. Lower P2Y12 Reactive Unit (PRU) at PCI and less calcified lesions were noted to be independent prognostic factors for IS-Th resolution. It was suggested that local lesion-related factors and lower on-treatment platelet reactivity at the time of PCI could assist in the resolution of IS-Th following everolimus-eluting stent implantation, with the potential to improve clinical outcome.
