Long-term reintervention after endovascular abdominal aortic aneurysm repair

Columbo JA, Martinez-Camblor P, O’Malley AJ, et al. - Researchers investigated the long-term reintervention rate after endovascular abdominal aortic aneurysm repair (EVR), as well as determined the predictive factors for reintervention. Using the Vascular Quality Initiative registry linked to Medicare claims, they identified 12,911 patients who underwent EVR [mean age 75.5 ± 7.3 years; 79.9% male; 89.1% of operations elective]. They identified a 3-year reintervention rate of 15%, and 10-year rate of 33%. Following five factors were identified as predictive of reintervention: operative time ≥ 3.0 hours, aneurysm diameter ≥ 6.0 cm, an iliac artery aneurysm ≥ 2.0 cm, emergency surgery, and a history of prior aortic surgery. Overall findings suggest a high likelihood of reintervention after EVR among all patients. However, clinical factors at the time of repair can aid in predicting those at highest risk.
