Long-term outcomes after open reduction and internal fixation of bicondylar tibial plateau fractures

Ochen Y, Peek J, McTague MF, et al. - Researchers aimed at establishing normative data, long-term patient-reported functional outcome and health-related quality of life (HrQoL) following operative treatment of bicondylar tibial plateau fractures. Further, they sought risk factors linked with functional outcome and HrQoL. In this retrospective cohort study, all adult patients with AO/OTA 41-C or Schatzker V/ VI tibial plateau fractures who underwent treatment between 2001 and 2016 (n = 450) by open reduction internal fixation were reviewed. Among these cases, 26 suffered infection (12%) and 6 patients (3%) received total knee arthroplasty (TKA). A minimum clinically important difference was not reached for the PROMIS Physical Function (PROMIS PF) and EuroQol 5-Dimensions 3-Levels. No difficulty in walking more than a mile or climbing a flight of stairs was evident in patients as per the PROMIS PF items. However, patients experienced difficulty in doing vigorous activities and patients should be counseled concerning the anticipated long-term outcomes. As per the multivariable regression model, worse functional outcome was observed in correlation with female gender, diabetes, and worse HrQoL. The multivariable regression model revealed worse HrQoL in correlation with smoking, diabetes, and the subsequent need for TKA.
