Intraosseous regional prophylaxis provides higher tissue concentrations in high BMI patients in total knee arthroplasty: A randomized trial

Chin SJ, et al. - Herein the researchers compared tissue concentrations of low-dose vancomycin via intraosseous regional administration (IORA) vs actual body-weight adjusted systemic IV dose in primary total knee arthroplasty (TKA). Low-dose IORA was shown to be effective in the high-BMI population group, providing tissue concentrations of vancomycin 5-9 times higher than systemic administration. Results demonstrated that with IORA, the timing of vancomycin administration was optimized. IORA also provided high tissue antibiotic concentrations during TKA in this high-risk patient group.
