Intersecting experiences of healthcare denials among transgender and nonbinary patients

American Journal of Preventive Medicine Feb 04, 2020

Kattari SK, et al. - In this study performed on transgender and nonbinary population, researchers assessed the increased probability of facing denials of trans-related care and standard care across socioeconomic and identity characteristics in these populations, including race, age, educational attainment, disability, income, and gender identity. For this purpose, they used data from the 2015 U.S. Trans Survey ( n = 27,715) as well as multivariate logistic regressions. Denials of trans-specific healthcare had been encountered by approximately 8% of the participants, and > 3% had been denied general healthcare. The probability of facing denial of care in general or trans-specific healthcare settings was significantly more among transgender (vs nonbinary), older, biracial, or multiracial, and lower-income participants, as well as those with less than a high school diploma and those with disabilities. Findings call for better training of healthcare providers so that they may be inclusive and decrease refusal rates of their transgender and nonbinary patients. However, it is important to consider current rates of denial through a whole-person lens, taking into account the experience of concurrent oppressed identities and acknowledging the raised risk those with multiple marginalized identities experience in being refused needed healthcare.
