Infective endocarditis hospitalizations and outcomes in patients with end‐stage kidney disease: A nationwide data‐linkage study

Journal of the American Heart Association Oct 01, 2021

Gallacher PJ, McAllister DA, Mills NL, et al. - A poor prognosis linked with infective endocarditis (IE) was evident in end‐stage kidney disease (ESKD) patients, especially in the longer term. ESKD patients, relative to those without ESKD, were twice as likely to die within 1 year, and 3 times as likely to die within 3 years of IE hospitalization.

  • This retrospective, population‐based cohort study included patients with ESKD requiring dialysis or with a kidney transplant.

  • Between 1990 and 2014, there occurred 7,638 incident IE hospitalizations; 2.8% in 210 patients with ESKD and 97.2% in 7,303 patients without ESKD.

  • There were positive findings from blood cultures in 42% (950/2,267) of incident IE hospitalizations from 2008.

  • In patients with and without ESKD, Staphylococcus aureus was isolated in 25.9% (21/81) and 12.8% (280/2,186), respectively.

  • ESKD was related to an elevated odds of death at 1 (44.9% vs 31.4%; adjusted odds ratio [aOR], 2.47) and 3 years (63.9% vs 42.8%; aOR, 3.77).
