Hypothermic oxygenated perfusion vs normothermic regional perfusion in liver transplantation from controlled donation after circulatory death: First international comparative study

Muller X, Mohkam K, Mueller M, et al. - In view of the higher rates of graft loss in correlation with liver transplantation following cDCD and the possibility that preservation strategies like NRP and HOPE may offer safer use of cDCD grafts, researchers compared HOPE and NRP in liver transplantation from cDCD. In this retrospective comparative cohort study, outcomes following cDCD liver transplantation in 1 Swiss (HOPE) and 6 French (NRP) centers were assessed. They identified transplantation of a total of 132 and 93 liver grafts after NRP and HOPE, respectively. The posttransplant recipient and graft survival rates seen with NRP and HOPE in cDCD were comparable; these rates exceeded 85% and were similar to benchmark values noted in standard DBD liver transplantation. As a consequence of procurement of grafts in the HOPE cohort being from older donors and their longer warm ischemia times, higher graft utilization rates were observed.
