Hypertonic mannitol for the prevention of intradialytic hypotension: A randomized controlled trial

Mc Causland FR, et al. - In this double-blind, single-center, randomized, controlled trial including subjects needing initiation of hemodialysis (HD) for acute or chronic kidney disease, researchers assessed how the occurrence of intradialytic hypotension (IDH) was influenced by an intervention to minimize intradialytic alterations in plasma osmolality. During the first 3 HD sessions, they administered mannitol (0.25 g/kg/h) vs a similar volume of 0.9% saline solution. Findings revealed that absolute systolic blood pressure decline did not differ between those who received mannitol and those who received saline solution. However, the mannitol group had fewer overall IDH events as well as a nominally lower risk for dialysis sessions being complicated by IDH.
