Hospital-level disparities in the outcomes of acute myocardial infarction with cardiogenic shock

Vallabhajosyula S, et al. - Using a retrospective cohort, researchers focused on hospital-level differences in the outcomes of acute myocardial infarction complicated with cardiogenic shock (AMI-CS). From the National Inpatient Sample database (2000-2014), adult admissions with a primary diagnosis of AMI and concomitant CS were selected, after excluding inter-hospital transfers. More frequently male, younger patients with lower comorbidity and higher illness severity sought admissions to urban and large hospitals. Lower mortality was reported in relation to admission to an urban non-teaching hospital and urban teaching hospital vs rural hospitals. Lower in-hospital mortality was reported in relation to admission to a large hospital vs a small hospital. Greater use of early coronary angiography, percutaneous coronary intervention, mechanical circulatory support was reported in large and urban hospitals. Overall, management and outcomes of AMI-CS differed across various hospital categories and the observed differences did not fully account for variations in patient features.
