Gabapentin and pregabalin use and association with adverse outcomes among hemodialysis patients

Ishida JH, et al. - Researchers explored the link between gabapentin and pregabalin use (modeled as separate time-varying exposures) and adverse outcomes (time to first emergency room visit or hospitalization for altered mental status, fall, and fracture) in Medicare-covered adults receiving hemodialysis with Part D coverage in 2011, identified from the US Renal Data System. They found that use of gabapentin was related to 50%, 55%, and 38% higher hazards of altered mental status, fall, and fracture, respectively, in the highest dose category. They also noted a higher hazard of altered mental status and fall in association with even lower dosing of gabapentin. Up to 51% and 68% higher hazards of altered mental status and fall, respectively, were reported in association with pregabalin use. Overall, judicious use of gabapentin and pregabalin in patients on hemodialysis was recommended.
