Factors associated with advanced colorectal neoplasia in patients with CKD

Au EH, Wong G, Howard K, et al. - This prospective cohort study revealed the following as factors linked with advanced colorectal neoplasia in patients with chronic kidney disease (CKD): older age, male gender, erythropoiesis-stimulating agents and azathioprine.

  • As part of the Detecting Bowel Cancer in CKD (DETECT) Study, 1,706 patients with CKD stages 3-5, including those treated with maintenance dialysis or transplantation, underwent screening for colorectal neoplasia using a fecal immunochemical test (FIT).

  • Advanced colorectal neoplasia was detected in 117 of these patients (6.9%) (54 with CKD stages 3-5 not receiving dialysis, 34 receiving dialysis, and 29 recipients of a transplant), including nine colorectal cancers.

  • Advanced colorectal neoplasia was identified in correlation with factors like older age (odds ratio (OR) 1.05 per year), male gender (OR 2.27), azathioprine use (OR 2.99) and erythropoiesis-stimulating agent use (OR 1.92).
