Environmental and socioeconomic factors influence the live‐born incidence of congenital heart disease: A population‐based study in California

Peyvandi S, Baer RJ, Chambers CD, et al. - Among live‐born infants in California, researchers assessed the incidence of congenital heart disease (CHD) in relation to socioeconomic as well as environmental factors in this population‐based cohort study. They also investigated if maternal comorbidities are in the causal pathway. Socioeconomic status and environmental exposure to pollutants ascertained by US Census data were included as predictors. Experts assigned a social deprivation index and environmental exposure index, on the basis of neighborhood socioeconomic variables, classified into 4 quartiles. Quartile 1 represented the best with the least exposure to pollutants as well as social deprivation, and quartile 4 represented the worst. CHD incidence among 2,419,651 live‐born infants was estimated to be 3.2 per 1,000 live births. A significantly higher incidence was observed among those in quartile 4 vs quartile 1 following adjustment for maternal race/ethnicity and age and accounting for the link between the 2 primary predictors. Overall, findings revealed an association of increased social deprivation as well as exposure to environmental pollutants with the incidence of live‐born CHD in California. Some, but not all, of this link was explained by maternal comorbidities.
