Early termination of resuscitation in in-hospital cardiac arrest and impact to the outcome calculations

Loisa E, et al. - In this 3-year prospective observational Utstein-style study, researchers focused on early termination of resuscitation—defined as resuscitation attempts that were terminated within 10 minutes—in in-hospital cardiac arrest, and evaluated the influence on the outcome calculations. Participants included 199 in-hospital cardiac arrest patients. Findings revealed that the presumed futility of the attempt led to the termination of one-seventh of resuscitation attempts. Non-witnessed arrest presenting asystole as initial rhythm and severe acute illness were documented as the most frequent causes for termination decisions. Removal of early terminated attempts from the outcome analyses let short- and long-term outcomes to be 5% and 4% better. The investigators concluded that in-hospital cardiac arrest outcomes were not as poor as reported in the literature.
