Delays to anti-tuberculosis treatment intiation among cases on directly observed treatment short course in districts of southwestern Ethiopia: A cross sectional study

Asres A, et al. - Researchers performed a cross-sectional study among 735 new adult tuberculosis (TB) cases in 10 districts of southwestern Ethiopia to assess patient and provider related delays to diagnosis and treatment of TB. Observations revealed the median (inter-quartile range) of patient, provider and total delays of 25 (IQR;15–36), 22 (IQR:9–48) and 55 (IQR:32–100) days, respectively. Tuberculosis patients in the study area waited too long to begin anti-TB treatment, indicating longer periods of morbidity and disease transmission. Higher odds of patient delay had an independent correlation to prior self-treatment, HIV co-infection, and extra-pulmonary TB, while longer provider delay was significantly related to initial presentation to health posts or private clinics and patient delay. A longer total delay could be independently predicted with having extrapulmonary TB, previous consultation of traditional healer, and use of holy water. These findings suggest the necessity for multifaceted intervention at all levels, so health extension workers and TB treatment supporters can decrease the patient delay. Improving community awareness, involving informal providers, enhancing cough screening and improving diagnostic efficiencies of healthcare facilities can lessen provider delays.
