Continuity and utilization of health and community care in elderly patients with heart failure before and after hospitalization

Safstrom E, et al. - In this cross-sectional study, researchers reviewed patients’ perspective and medical charts to determine health and community care use before and 30 days after hospitalization, and the continuity of care in patients hospitalized due to de novo or deteriorated heart failure (HF). They noted no visits to any health care facility within 30 days before hospitalization among most patients. After hospitalization, significant increase was observed in health care use. Despite receiving written information at discharge by most patients, one third had no idea regarding which health care provider to contact in the event of deterioration and these subjects felt insecure at home after discharge, indicating flaws in the continuity of care.
