Comparative outcomes of transthoracic vs transhiatal esophagectomy

Takahashi C, Shridhar R, Huston J, et al. - Since surgical resection has become a mainstay of therapy for locally advanced esophageal cancer and can increase survival significantly, researchers reported a modern analysis of outcomes with transthoracic vs transhiatal esophagectomy. Between 1996 and 2016, a prospectively managed esophagectomy database was queried for patients undergoing transthoracic or transhiatal esophagectomy. In total, 846 patients underwent esophagectomy with a median age of 66 (28–86) years. The authors discovered that a transthoracic approach was related to lower pneumonias, anastomotic leaks, wound infections, and strictures, with an improvement in nodal harvest. In addition, survival was significantly improved in patients who had transthoracic esophagectomy.
