Comorbid monosymptomatic nocturnal enuresis and snoring exhibit an additive effect on impairments in heath-related quality of life

Wolfe-Christensen C, et al. - A retrospective chart review was done in an outpatient pediatric urology clinic as well as in an outpatient pulmonology clinic by the researchers in order to ascertain whether the combination of monosymptomatic nocturnal enuresis (MNE) and sleep-disordered breathing (SDB) results in reduced health-related quality of life (HRQOL) in a pediatric sample (n = 152). Patients were classified as either, MNE Only, SDB only, MNE + SDB, or healthy control. Significantly more severe SDB was noted in patients in the SDB only group vs the other groups. On all subscales of the OSAS-18, the four groups significantly varied, ie, Sleep Disturbance, Physical Symptoms, Emotional Symptoms, Daytime Functioning, and Caregiver Concerns. Post hoc analyses exhibited that there were more impairments on each subscale in the MNE + SDB group. Analysis of the total impairments revealed that children with only one condition (MNE or SDB) reported comparable levels of impairments in HRQOL, patients with MNE + SBD reported significantly greater levels of impairments, as it seemed that these comorbid conditions had an additive impact on HRQOL. Thus, children with comorbid MNE and SDB are at significantly greater risk for reporting impairments in their HRQOL in comparison with children who only have one of these conditions. While it has already been known that children with MNE are at risk for emotional, behavioral, and social difficulties, and children with SDB are at risk for neurobehavioral complications, it is likely that children with both conditions are at risk for a multitude of negative consequences. These data highlight the importance of urologists screening for SDB and pulmonologist screening for MNE as part of their routine clinical practice.
