Clinical prognostic factors and outcome in pediatric osteosarcoma: Effect of delay in local control and degree of necrosis in a multidisciplinary setting in Lebanon

Ali BA, Salman M, Ghanem KM, et al. - Given a dramatic improvement in pediatric osteosarcoma outcomes during the past few decades, researchers performed a retrospective review among 38 patients managed between 2001 and 2012 at a single institution in Lebanon, to study clinical features as well as outcomes of these patients. A uniform three-drug chemotherapy regimen comprising cisplatin, doxorubicin, and methotrexate was used to treat all patients. Cases with metastatic disease and/or poor degree of tumor necrosis (< 90%) were given ifosfamide and etoposide added to the adjuvant treatment regimen. A median follow-up of 61 months was performed. The estimated 5-year overall and event-free survival rates in patients with localized disease were about 81% and 68%, respectively, and these were approximately 42% for metastatic disease. The observed survival among pediatric osteosarcoma patients treated in a multidisciplinary cancer center in Lebanon was similar to that reported in developed countries. The worse outcome was seen in relation to the delay in local control. Poor degree of necrosis at the time of local control was the only statistically significant inferior outcome predictor that was identified in this study.
