Clinical outcomes in patients with biopsy-proved diabetic nephropathy compared to isolated lupus or crescentic glomerulonephritis

Diabetes Research and Clinical Practice Feb 24, 2019

Chou YH, et al. – In this study, investigators analyzed the outcome of patients with biopsy-confirmed diabetic nephropathy (DMN) vs patients with isolated crescentic glomerulonephritis (GN) or lupus nephritis (LN). Participants in the study included 55 DMN, 48 crescentic GN, and 82 LN patients from an original cohort of 987 adults who underwent kidney biopsy. To identify factors related to the outcome measures of end-stage renal disease (ESRD) and all-cause mortality, they used the Cox regression model. Patients with DMN and crescentic GN demonstrated elevated rates of ESRD vs LN patients. Independent predictors for ESRD included DMN vs LN, lower hemoglobin values, lower estimated glomerular filtration rates, and severe proteinuria, after adjusting for the competing risk of death. Patients with DMN and crescentic GN demonstrated mortality rates vs LN patients following ESRD development. Upon multivariate analysis, old age (≥ 65 years) and lower serum albumin levels were found to be independently associated with overall death.
