Blood pressure control and cardiovascular outcomes in patients with atrial fibrillation (from the ORBIT-AF registry)

Vemulapalli S, et al. - Among 10,132 patients with atrial fibrillation (AF) from the ORBIT-AF registry, researchers assessed the prevalence of baseline uncontrolled hypertension and the overall variation in systolic blood pressure (SBP) control. They also investigated factors that predicted uncontrolled SBP over 2 years of follow-up, as well as the link between SBP and clinical results. Participants were categorized as those with no history of hypertension, those with controlled hypertension (baseline SBP <140 mmHg), and those having uncontrolled hypertension (baseline SBP >140 mmHg). A total of 8,383 (83%) patients had hypertension, with 2,032 (24.2%) having uncontrolled baseline SBP; over 2 years little change was seen. Uncontrolled baseline SBP, being female, previous percutaneous coronary intervention, and having diabetes were identified as predictive factors of elevated follow-up SBP. Increasing adverse events were observed in association with higher SBP among patients with AF, highlighting the need for more rigorous blood pressure control.
